Payment Policies

I understand that there is an annual registration fee. I understand my spot in the class is not guaranteed until the first payment is received. I understand there is a $10 late fee if payment is not received within 3 days of the due date. If payment is not received, the student may be dropped from the class. I understand that all fees are non-refundable. I understand I must be on location to pick up my child by the time their class ends. I understand that if I am late for pick up, I may be charged a late pickup fee at $1 per minute. I understand there are no refunds/cancellations for classes once that month has begun/been paid for. I understand there are no make-up classes as some months have 5 classes instead of 4. I understand if I join during the current month, my payments will be prorated to reflect the amount of classes I will have. I understand there is a small Transaction/Processing fee to occur during my monthly payments. I understand, if I wish to unenroll my athlete for the following month, I must give a notice at least two weeks before next payment is due, or I will be responsible for that next month’s payment 

Photo Release

I grant to LLC Angels Gymnastics Academy the right to take photographs/videos of my child. I agree that LLC Angels Gymnastics Academy may use such photographs or videos for any lawful purpose, including; publicity, illustration, advertising and web content. I understand I will not post any information that may compromise LLC Angels Gymnastics Academy. I will use discretion when posting social media in which the company, or other student’s information is displayed. Personal information of others must be removed prior to posting on social media. I understand that Parents and Students should never post negative comments about other students, teachers, parents, or facilities. 

Release of Liability

By reading and signing this form, you are agreeing and aware that you are engaging in activities with LLC Angels Gymnastics Academy, that include but are not limited to physical exercise and use of exercise equipment, facilities, training and instruction. These activities could cause injury to you or others. You are voluntarily participating in these activities and assume all risks of injury and liability that may result. 

You agree to waive any claims or rights you might otherwise have, to pursue Legal or other action against LLC Angels Gymnastics Academy, the facility's owners for any reason. It is advisable to consult your physician before undertaking a physical exercise program. You have carefully read this waiver and declare you are sufficiently physically fit for any exercise activities. 

Consent info/waiver

I understand LLC Angels Gymnastics Academy does not accept responsibility for injuries sustained on the premises before, during, or after attending classes, whether injury is intentional, accidental or resulting from activities or any other cause. We will do everything possible to ensure safety. LLC Angels Gymnastics Academy has taken reasonable measures to prevent the spread of infectious disease However, it is still a possibility to come in contact with infectious pathogens. I, the undersigned parent or guardian of the above named student, hereby grants authority to the staff of LLC Angels Gymnastics Academy to render judgment concerning medical assistance in the event of an accident or illness during my absence including but not limited to; administering first-aid care of minor injuries. I hereby authorize Angels Gymnastics Academy owners and employees to take action as necessary for the well-being and safety of the student in making medical decisions when I am unable to make arrangements for medical attention, such as transporting the student to a medical facility.  Please accept this form as authority to use the doctor on call in the emergency room for any necessary emergency medical treatment. 

Concussion Protocol 

All owners and employees of LLC Angels Gymnastics Academy must undergo a concussion awareness training program. Angels Gymnastics Academy follow the USAG guidelines and protocols for sports related concussions. https://static.usagym.org/PDFs/About%20USA%20Gymnastics/wellness/concussionpolicy.pdf

Class Placement Policies

I understand that LLC Angels Gymnastics Academy has the right to place athletes into the class that is best suited for them, regardless of desired class. I understand my child may be placed in a lower or higher level class at any time due to, but not limited to the following: Behavior and listening ability, Maturity, or Physical ability. I understand that these listed above can pose as safety hazards. I agree to allow coaches to make decisions on best fitting classes for my child. I understand that the company has the right to refuse service to anyone, and can revoke your membership with LLC Angels Gymnastics Academy at any time.

Makeup class/Missed class policies

I understand there are no refunds for missed classes. I understand there are no make-up classes offered since some month's offer 5 classes instead of 4. 

Parent-Teacher Communication 

Group me may be used for any direct communication between Teacher/Parents. I understand there is to be no direct communication between student and teacher, unless the parent is involved in the direct communication. 

Facility and Equipment Usage

I understand that I am welcome to stay in the lobby during my child’s class. I understand that in order to drop my student off My child must be able to use the restroom on their own. I understand I must pick up my child promptly once their class is over, or a late charge of $1 a minute will be charged to my account. I understand that LLC Angels Gymnastics Academy has classes back to back, and is not responsible for your student before or after designated class times. I understand that if my child is under the age of 7, I must come inside the building to pick them up from class. I understand the staff of Angels Gymnastics Academy will not be able to assist students with using the restroom. Restroom usage and Potty accidents are the parent’s responsibility. I understand and agree that the facility and equipment is to be used with respect, and any misuse of equipment may result in injury.
